Customized In-Home Supports bring one-on-one care into the individual's own home. From daily living assistance and personal care to building routines, independence, and safety, CIHS meets each person where they are — supporting greater confidence and quality of life in a familiar environment.
Who qualifies: Individuals on the NM DD Waiver with in-home support authorized in their service plan.
Customized Community Supports help individuals participate meaningfully in community life. Whether building social connections, developing daily-living and self-advocacy skills, or exploring interests and volunteering, CCS is designed entirely around each person's goals and preferences.
Who qualifies: DD Waiver participants with community engagement goals in their ISP.
Caring for a loved one is rewarding — and exhausting. Respite provides families and primary caregivers with trusted, temporary relief so they can rest, work, or attend to other responsibilities, knowing their loved one is in safe, capable hands.
Who qualifies: Individuals on the DD Waiver whose families or caregivers need authorized respite.
Family Living offers a nurturing, home-based residential model where an individual lives with a trained provider in a family setting. With clinical and compliance oversight, this high-touch service delivers stability, belonging, and daily support in a true home environment.

Our nursing oversight ensures health and safety remain at the center of care. From assessments and medication oversight to health coordination and documentation review, our clinical team helps individuals stay healthy and supports staff in delivering safe care.
